DR. DELEEUW: Any surgical procedure requires an incision and for breast augmentation we would like to keep the incision small, we’d like the scars to be unobtrusive. There are different incisions you can use. The commonest one that I use is in the crease under the breast and I use that incision because you can do accurate surgery and patients have limited discomfort during recovery. But you can also make an incision at the areolar border which usually heals with a very fine scar, and you can do excellent surgery through that incision, and you can make incisions in the armpit, or you can make incisions in the bellybutton which will leave scars which are not on the chest.

The issue of size and symmetry is always important in breast augmentation. Everyone has to realize that we’re all asymmetrical so our right side never matches our left side but they should be very close. You should only notice that asymmetry with a very studied look. The way we size patients is at their pre-operative visit they put test implants in their bra. They get to look at themselves in the mirror with a tight shirt, they can look at themselves with a bra, they can try multiple implants, they can spend an hour looking and deciding on the perfect size for them, and of course we give them advice as well, which is based on their body habitus, the base diameter of their breasts, how tall they are, how much they weigh, and factors like that. Ultimately they come to an informed decision that they make about the size of the implant.
